First identify immediate instability and safety threats. Then compare airway, breathing, circulation, acute change, and the scenario’s specific context.

Use an airborne infection isolation room when available and a fit-tested NIOSH-approved N95 or higher-level respirator. Add Standard Precautions.

Wear a mask upon room entry and use appropriate patient placement. If transport is necessary, the patient should wear a mask when tolerated.

Wear gown and gloves for interactions involving the patient or environment. Use dedicated or disposable patient-care equipment when possible.

Consider patient stability, predictability, task complexity, staff competency, supervision, and applicable law or facility policy. The nurse retains accountability.

Recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action, and evaluate outcomes. Reassess when the situation changes.
